Coming to class on your scheduled date and time automatically affirms you do not have any symptoms of an illness or any unexplained rashes within the past week. I understand that coming to Heart to Heart Career Training Center with symptoms will be exposing all the staff and fellow students to my illness and cause the spread of the disease. You will be asked to leave immediately and not be able to return until a healthcare professional has given you a letter stating you are free from disease.

I understand the above and acknowledge that I am disease free and will not attend classes if I have any symptoms.
